I lived it 12 hours a night working as a med tech on the dementia and hospice ward. I had a woman in a room dying and the only thing I could do for her was 3 drops of Morphine on her tongue every few hours. It may as well have been water. It did nothing for her. She suffered. The other 15 patients had to listen to it. Their visitors. The vendors that came through on the day shift. And those of us that did 12 on and 12 off. It's been about 5 years ago and I can still hear her scream.
